Upon receipt of the HFHW, the staff of "GENERAL CHEMICAL APPLICATIONS" issues/completes the following documents:
Weighing sheet
During collection, the waste is weighed on the scales of the Health Unit (HU) or, in the event that the HU does not have a scale, on a calibrated scale carried by each vehicle and in accordance with the procedure followed by each Health Unit.
The weighing sheet bears the name of the Health Unit, the date and time of receipt, the weight of the HFHW and the number of packages received.
Hazardous Waste Identification Document
During collection and transport, HFHW, which are transported for treatment outside the Health Unit, are accompanied by an identification form for the collection and transport of hazardous waste, which is completed (based on the model of JMD 146163/1537/2012) in collaboration with the person in charge of the health unit, whose record is kept, as proof of the management of HFHW. This form will accompany the waste in every collection and transport operation within the national territory, as well as at every stage of management, and will have the form presented in par. 6.2 of Annex I of JMD 146163/1537/2012.
